You, obviously start off on level 1 and chomp all the fruit you see on the grid, you grow bigger as you eat, so you have to be careful which route you take. If not, you might run into your own tail. When that happens .. you lose.
Eventually, after eating so much, the worm gets an adrenaline rush and the game progressively gets faster and faster. So it can become quite challenging.
